1. Our Commitment To Privacy

Please note that this Privacy Policy relates to your use of the 909 Healthcare website located at 909healthcare.com and your use of the 909 Healthcare Platform ("909 Healthcare platform") which includes CoV19™ prognostic screening tool survey, CoV19™ prognostic screening tool survey dashboard and any additional materials and applications. This Privacy Policy does not identify the policies of third-party web pages or third-party services that you may separately access through the site or the application (for example, online software application stores). Please consult the policies of those companies or sources to determine how they treat your private information.
We may modify this End User Agreement from time to time. We will take commercially reasonable efforts to notify registered users of our 909 Healthcare platform when we have modified the End User Agreement so that you can review the terms. No modification will be effective unless in writing and accepted by you and 909 Healthcare, which may be made available electronically through the 909 Healthcare website and/or the 909 Healthcare platform or other electronic means made available from 909 Healthcare, Inc.
2. The Information We Collect and How We Use It.
This notice applies to all information that may be collected or submitted on the Site or otherwise as part of your licensed use of the 909 Healthcare platform available exclusively at 909healthcare.com. When you register as a user on the software application you are required to provide us the following information as part your "account profile":

- Confirmation Password
*This information is ONLY used to correlate specific COVID-19 statistics (i.e. Centers for Disease Control) with individual user information to more accurately predict the potential infection of COVID-19, Influenza and Influenza-like illness.
We may also collect any other information that you include in the body of your message to 909 Healthcare including information about how we can contact you to respond to your request. Such information may include your phone number or mailing address.
3. Application Use and Aggregate Data Use
When you use the 909 Healthcare website or the 909 Healthcare platform you give us consent to and collect information about the time, date, geographic location and frequency of use of the 909 Healthcare website or the 909 Healthcare platform to generate aggregate data. We use the aggregate data generated for internal research, product and service development, our own marketing and quality control. Additionally, we may share or sell the non-personal, non-identifying, aggregate data that we collect with outside third parties. We will not provide any personally identifiable information to any third parties.
4. Information You Provide
Primarily, we use the information you provide about yourself when registering to complete the registration, to track the number of and types of users of the 909 Healthcare website or the 909 Healthcare platform, to target messaging to you, or to facilitate the internal messaging services between users and their respective organizations using the 909 Healthcare website or the 909 Healthcare platform.

As part of your registration to use the 909 Healthcare platform, you have also authorized us to communicate with you concerning upgrades, bug fixes and/or enhancements to the 909 Healthcare platform application, changes to the Privacy Policy and End User License Agreement or violations we identify or concerns we have related to your use of the 909 Healthcare website or the 909 Healthcare platform. We may also use your email address to send you additional information about us and our products and services. You may opt-out of receiving such emails by contacting us and requesting to be removed from the list. On rare occasions, it is necessary to send out a strictly service-related announcement, if, for instance, our service is temporarily suspended for maintenance. Generally, you may not opt-out of these communications, though you can cancel or terminate your registration. However, these communications are not promotional in nature. Your e-mail address is not used for any other purpose and is not shared with outside parties.

For that reason, we do not collect or maintain information at our website from those we actually know are under 13, and no part of our website is structured to attract anyone under 13 years of age. You may not register to use the 909 Healthcare platform if you are under 18 years of age.
7. How You Can Access, Correct or Remove Your Information
We use your e-mail address and your name to facilitate verifying your login, responding to your email requests, facilitating the transmission of messages between you and other users, and to provide information to you regarding upgrades, bug fixes and/or enhancements, or other additional information about us as noted previously. If your name, e-mail address or other user information changes, and you would like to continue receiving that information please contact us at support@909healthcare.com To protect your privacy and security, and to avoid misappropriation of the 909 Healthcare platform, we may take reasonable steps to verify your identity before permitting or implementing changes not initiated by healthcare provider administrators. If you no longer want us to collect, store or use your information, you must discontinue your use of the 909 Healthcare platform, cancel your account and send us a message at support@909healthcare.com requesting that we remove your information from our database. We will use commercially reasonable means of removing your information within a reasonable time of receiving your request so long as you have cancelled your account and cease use of the 909 Healthcare platform, however, certain non-personal, non-identifiable, aggregated information or data that we collected from you may not be removable.
